Information You Provide
The website’s native forms collect your contact name, company, email, phone, selected coverages and any optional information you provide. The septic intake can also collect business operations, business EIN, payroll, safety procedures, vehicle and crane details, and other underwriting answers. Do not enter Social Security numbers, driver’s license numbers, payment details or medical information. Arrange sensitive documents directly with Jesse.
How Inquiries Are Used
Your information is used for the insurance review and follow-up you request. It may be shared with insurance providers as needed to evaluate your business. Septic answers are mapped into draft underwriting forms for broker and applicant review. An inquiry does not sign an application, bind coverage or automatically submit documents to an insurer.
Storage And Notifications
Inquiries are saved in the website’s hosted database. Access to the broker inbox and supplemental downloads is restricted to authorized accounts. Hosting and approved email service providers process information to operate the website and support notifications to Jesse. The notification connection supports Resend; notification messages contain contact and coverage details with a link to the private inquiry record. Earlier Jotform inquiries remain with that service; new native-form inquiries are saved on this website.
Campaign Information
If a campaign link includes source or campaign tags, those tags are saved with the inquiry to help Jesse understand how you found the website. The site also uses a daily-changing hash of the network address, when available, to limit repeated submissions. It does not add advertising tracking cookies. Unsubmitted form entries are kept in the current page’s memory and are lost if you close or refresh it.
External Services
Booking links open Calendly. Email links open your email application. Authorized broker sign-in uses ChatGPT. These services handle information under their own terms and privacy policies.
